Privacy Notice Statement: Regarding Personally Identifiable Information (PII)

 

This Privacy Notice Statement is required and explains how Holistic Quote LLC (“we”/”us”) may collect, use and disclose personally identifiable information (“PII”).

Personally identifiable information (PII), defined by the Office of Management and Budget (OMB), refers to information that can be used to distinguish or trace an individual's identity, like their name, Medicare Number, biometric records, etc. alone, or when combined with other personal or identifying information which is linked or linkable to a specific individual, like date and place of birth, mother’s maiden name, etc. Medicare Fee-for-Service eligibility and enrollment information and claims data are considered protected health information (PHI) under the Health Insurance Portability and Accountability Act (1996) (HIPAA) regulations.

We will take reasonable steps to ensure that any of your PII that we have on file is complete, accurate, and up to date. 

We will obtain the consent of an individual, employer, or employee prior to helping them apply for financial help and/or enrolling in a Marketplace qualified health plan (“QHP”). We agree to take reasonable steps to ensure PII provided to the Marketplace is accurate and that the PII has not been altered or destroyed in an unauthorized manner, thereby ensuring the confidentiality, integrity, and availability of information.

Legal Authority To Collect PII

We have entered into an agreement with the Marketplace. We comply with the Marketplace’s privacy and security standards established by the U.S. Department of Health and Human Services and the Centers for Medicare & Medicaid Services. We may collect, use, or disclose your PII if and to the extent required under applicable law. In all other instances, we may collect, use, or disclose your PII, in accordance with this Privacy Notice Statement in order to assist with application, eligibility, plan selection, enrollment, renewal, and disenrollment activities, assist consumers with applying for APTC and CSR eligibility determinations, educate consumers on Insurance Affordability Programs, Medicaid, and the Children’s Health Insurance Program (CHIP), if applicable, and to meet legal responsibilities and conduct quality assurance.

 

Purpose of the Information Collection

The purpose of our collection of PII is to assist with application, eligibility, plan selection, enrollment, renewal, and disenrollment activities, assist consumers with applying for APTC and CSR eligibility determinations, educate consumers on Insurance Affordability Programs, Medicaid, and the Children’s Health Insurance Program (CHIP), if applicable, and to meet legal responsibilities and conduct quality assurance.

 

To whom PII may be disclosed, and for what purposes

We may disclose your PII to the Marketplace (or employees, agents or subcontractors of the Marketplace), certain federal or state agencies, a health insurance issuer offering a QHP that you have selected (or such issuer’s employees, agents, or subcontractors), or any individual who is legally authorized to act on your behalf in connection with your PII. This includes any individuals acting under an appropriate power of attorney or appointment that complies with applicable state and federal law. Your PII may be disclosed for us to assist with application, eligibility, plan selection, enrollment, renewal, and disenrollment activities, assist consumers with applying for APTC and CSR eligibility determinations, educate consumers on Insurance Affordability Programs, Medicaid, and the Children’s Health Insurance Program (CHIP), if applicable, and to meet legal responsibilities and conduct quality assurance.

 

Authorized uses and disclosures of any collected information:

We may use and disclose PII to to assist with comparing health insurance plans, obtaining determinations of eligibility for coverage, enrolling in QHPs through the Marketplace, assist with application, eligibility, plan selection, enrollment, renewal, and disenrollment activities, assist consumers with applying for APTC and CSR eligibility determinations, educate consumers on Insurance Affordability Programs, Medicaid, and the Children’s Health Insurance Program (CHIP), if applicable, and to meet legal responsibilities and conduct quality assurance.

 

Whether request to collect PII is voluntary or mandatory under applicable law

Our request to collect your PII is voluntary.

 

Effects of non-disclosure if an individual chooses not to provide the requested information

If you choose not to provide the requested PII, then we may be unable to assist with comparing health insurance plans, obtaining determinations of eligibility for coverage, enrolling in QHPs through the Marketplace, assist with application, eligibility, plan selection, enrollment, renewal, and disenrollment activities, assist consumers with applying for APTC and CSR eligibility determinations, educate consumers on Insurance Affordability Programs, Medicaid, and the Children’s Health Insurance Program (CHIP), if applicable, and to meet legal responsibilities and conduct quality assurance.
